Does Medicare cover Wegovy for heart disease?

Yes — but not the way most people think.

Medicare Part D doesn’t cover Wegovy for weight loss. It does cover Wegovy when it’s prescribed to lower the risk of heart attack, stroke, and cardiovascular death in adults who already have heart disease and are overweight or obese. The two situations look almost identical from the outside — same drug, same patient — but Medicare treats them very differently.

Since March 2024, Medicare Part D plans can cover Wegovy when it is prescribed to reduce the risk of heart attack, stroke, and cardiovascular death in adults with established cardiovascular disease and a BMI of 27 or higher. That is a separately approved FDA indication — not weight loss — and it is what makes Wegovy coverage legally possible under Part D.

Weight loss vs. heart-disease risk reduction.

Prescribed for weight loss alone

Federal law (the Medicare Modernization Act) bars Part D from covering drugs prescribed solely for chronic weight management. Without a separate qualifying diagnosis, Wegovy is out-of-pocket — typically $1,000–$1,400 per month.

The new GLP-1 Bridge Program (July 1, 2026) is the workaround for weight-loss-only patients.

Prescribed for cardiovascular risk reduction

In March 2024 the FDA approved Wegovy to reduce major adverse cardiovascular events (MACE) in adults with established cardiovascular disease and obesity or overweight. CMS quickly confirmed Part D plans can cover Wegovy under this cardioprotective indication.

Coverage here is treated as cardiac prevention — like a statin — not as weight management.

When Medicare covers Wegovy.

  • Wegovy for reducing risk of heart attack, stroke, or cardiovascular death in adults with established CVD + BMI ≥ 27

    Covered by Medicare Part D since March 2024. Prior authorization with documentation of qualifying cardiovascular disease is typically required.

  • Wegovy for chronic weight management without a qualifying cardiac diagnosis.

    Excluded by federal statute from Medicare Part D.

  • Wegovy for chronic weight management starting July 1, 2026

    Covered by the new federal GLP-1 Bridge Program at $50/month for qualifying beneficiaries.

How to qualify

WHEN DOES MEDICARE COVER WEGOVY?

Medicare Part D may cover Wegovy for cardiovascular risk reduction when all four of the following requirements are met.

You have Medicare prescription coverage

Wegovy is covered through Medicare's prescription drug benefit. To qualify, you must have Medicare and prescription drug coverage through a Medicare Part D or Medicare Advantage plan.

You have established cardiovascular disease

Documented prior heart attack, prior stroke, or peripheral arterial disease. In addition to meeting BMI requirements, Medicare coverage under this indication requires a history of cardiovascular disease.

Your BMI is 27 or higher

The FDA cardiovascular indication applies to adults who are overweight (BMI ≥ 27) or obese (BMI ≥ 30) in addition to having established cardiovascular disease.

Wegovy is part of your cardiovascular care plan

Medicare coverage under this indication is based on Wegovy's ability to reduce cardiovascular risk in eligible patients with heart disease and overweight or obesity—not solely its effects on weight.
